To ensure your privacy and secure your information, policies and procedures have been established by various agencies including the U.S Federal government through their Office of Civil Rights (HIPAA, HiTech), the State of Texas through their Legislature and The Texas Medical Records Practice Act (TMRPA or Texas House Bill 300) for your protection. Enforcement of these policies and investigation of complaints is performed by the Office Of Civil Rights and Various Texas Agencies including the Texas Attorney General Office and their Consumer Protection Division and The Texas Medical Board in the case of Physicians. Web Site Data
We monitor traffic to our web site and traffic to our newsletter. Traffic to our website cannot be tracked back to the visitor, however traffic to our newsletter which you have subscribed to and links from the newsletter can be tracked back to the specific visitor. We know how often our website and certain pages are visited and the IP address of the visitor but this information cannot reveal to us the actual identity of the visitor. This tracking is for making decisions on how and where to improve our website. Such queries are are also tracked for analysis of traffic flows to our site.

Contact Form is provided by Paubox.com and is a HIPAA Secure Encrypted way to communicate with us and send us documents such as intake forms. If you contact us through our Contact Form and provide us with Personal Information (such as your name, phone number, address and email address), we use that information to respond to your comments. We strongly urge you NOT to submit to us sensitive information, such as your Social Security Number or credit card information, unless it is necessary for the purposes of your communication. Minors should not submit personal information without the consent of their parent or guardian. However, there is no way for us to prevent minors from voluntarily furnishing us this information.Please contact us if you have any questions regarding personal information that your minor children may have provided us.
If you become a client you will be asked to sign an Authorization and Consent Form which gives us permission to continue to contact you by possibly unsecured text/voice/fax/email or internet communication channels as described in the Authorization and Consent Form.
Diffusion of information to third parties
Inside Outside Wellness Center & Medical Spa will not share, loan, or sell your PHI or ePHI you give us or send us when you use our Contact Form or any of our Intake Forms with any third parties. Authorization is required, by law or by you, to share PHI and ePHI with any third party.
